- Complete the eligibility form to confirm the participation criteria are met
- Instructions to access a scheduling form will be given. It may be used to determine when a virtual or in-person informed consent can be conducted with a member of the project team
By providing your contact information and submitting the form, you are providing your consent to be contacted by the genCARE project team at the participating centres
